If you want to revoke a power of attorney, we will tell you how to do it by creating a new power of attorney or canceling the old one.
Steps
1 Determine the cancellation method. You can simply revoke the previous power of attorney or create a new one. If you want to cancel a power of attorney, you need to fill out the "Cancellation of power of attorney" form. If you want to create a new power of attorney, do so, then the old power of attorney will be automatically canceled. 
2 Authorize the form. The only person who must authorize the power of attorney is the person who is the main subject of this power of attorney, that is, the person who entrusts certain powers to another person. You do not need to go to a notary to draw up such a document. 
3 Now the most important thing is to take your form and file it in the right place. In order for your bank, hospital or institution to know that you have revoked a previous power of attorney, you need to inform them about it. Send them a new power of attorney or a message to cancel the old one.

- Make sure that you do not forget to report the revocation of the power of attorney to all authorities.
