Pierce a hole from the top through the apple. Place the tip of the stylus from where you just removed the stem and swirl on the apple. Press the pen until you make a hole that spans about half the apple.

If you find it difficult to press the pen inside the apple, you can press and rotate the pen while pressing.

Push the pen into the apple until the third hole intersects with the hole system you made. This hole acts as a smoke vent when you smoke, so it should be conveniently located so you can cover the hole with your finger while you are filling the straw.

Create a medication hole. Use a pen or other sharp object to cut a hole in the top of the apple where you made the first hole. Swipe wide enough to fit the ingredients of the tobacco you want to smoke.

Rotate the spoon around the edge of the apple to create a circle about 1 cm from the edge of the apple. Press the spoon against the apple to remove the flesh, about 1 cm from the bottom of the apple.
- Don't cut too deeply into the edge or bottom of the apple when you use a spoon. The straw will not be stiff enough unless you leave enough apple to build a strong tube structure.
- Use a knife to cut the core of the apple in the middle of the circle if you can't use a spoon to cut the flesh off the apple. Cut the core of the apple several times if needed, then cut the apple around with a spoon.
